Uplift Carer Link
Long-term Stress and Social Isolation of Caregivers
As the proportion of families facing the situation of "elderly caring for elderly" and "disabled caring for elderly" continues to rise, caregivers face long-term stress and are prone to social isolation and mental health crises. They bear an indescribable burden, gradually becoming disconnected from society, leading to physical and mental exhaustion and creating a serious isolation dilemma.
As a caregiver, have you noticed any warning signs concerning your stress levels, daily coping abilities, or your own physical and mental health?

Introduction
Funded by the HKEX Charitable Foundation, Elderly Community Support and Care Services will pilot an innovative district-based support model in Tai Po District from December 2025 to November 2028. With Saint Francis University serving as our Academic Partner to conduct research, we are recruiting community members to form a "Caring Guardian Alliance" to identify high-risk caring families. By establishing community safety networks, promoting gerontech and innovation, strengthening the social connections and resilience, we aim to help caregivers rebuild social networks and embrace a new life.
Service target
- Caregivers residing in Tai Po District who are caring for frail elderly persons aged 60 or above.
- Including those who are elderly, disabled, or experiencing caregiving stress who are currently receiving insufficient or no formal community support services.
Project Objectives
- Establish a Community Safety Net
To proactively identify high-risk families and provide timely, comprehensive support for caregivers.
- Leverage Gerontechnology
To empower caregivers and the elderly to master basic smart technology applications, thereby enhancing their physical and mental well-being.
- Foster Social Connections
To rebuild caregivers' social networks through peer support groups and employment transition assistance.
