“Navigation for Closing the Carer Support Gap” Service

Program Introduction and Objectives: The project is supported by HSBC 150th Anniversary Charity Programme through a donation to The Community Chest of Hong Kong and launched in October 2022 for a period of three years. 
 
The project aims at training, developing and providing peers as "Navigators" to carers in meeting different needs along their caring journey.  They will help to signposting available services and community resources to users in need and befriending with them.  The project will also target elderly and disabled persons whose families have or will migrate overseas.  The Navigators, with the support of other stakeholders in the community, will provide continuous care, regular contacts and support to this specific group of clients.  The ultimate goal is to develop a salient social support network around clients in reducing the impact of social isolation and loneliness.-
 
Program content:
 
1. Provide effective assessments and action plans for carers in need
2. Train frontline staff and volunteers to become "Carers Support Navigator" to provide carers with adequate support
3. Promote "Advance Care Directives for Caregivers" to assist caregivers who will immigrate to formulate a plan for emergency care and emergency support
4. Provide training on "Advance Care Directives for Carers" for frontline staff and volunteers
5. Establish an interactive platform and contact more institutions to promote navigation services
 
Service Targets:
1. 7 categories of caregivers in need, including:
- Elderly Caregivers (Elderly Caregivers)
- Carers of Physically Disabled Persons carers of the mentally handicapped
- Mental health caregivers
- Carers of addicted persons (including drugs, alcohol and tobacco, drugs)
- Carers of children/persons with special learning needs
- Carers of chronically ill persons
 
2. Carers who will immigrate and leave Hong Kong and left-behind elders
