Dementia Hand in Hand


Project Period: October 2021 to September 2024 (36 Months)
Aims:
1) To enhance the quality of life of persons with dementia (PwD) and their caregivers;
2) To provide timely and appropriate support to the caregivers; and
3) To promote dementia and carer friendly community in the district
Target Group:
In Cho Yiu Chuen and Lai Yiu Estate, those supporting to or with dementia at early stage or mild cognitive impairment (MCI) and may be in need of respite and supportive care

About the Project:
1) Individual level:

2) Family Level: To enhance the quality of life of carers throughout the journey of caregiving by introducing carers’ need assessment, carers’ wellness plan, peer support program, use of technology and respite service.
3) Community Level: To build up dementia- and carer-friendly community, which understands, respects and supports the need of people with dementia and their carers.

Research Team: The Evaluation Study co-worked with Sau Po Centre on Ageing of The University of Hong Kong, aiming to:
1) evaluate the planned respite service (feasibility, acceptance, and impact on caregivers);
2) propose intervention strategies to reduce perception discrepancies between people with dementia and stakeholders;
3) evaluate the outcome of the intervention.
