The project is a two-year Self-Management Plan for Health program launched in our Neighbourhood Elderly Centre (NEC) to promote Active Aging of the health component. The plan is divided into three parts:
1S Stay Fit
The NEC promotes exercises that are designed by the physiotherapist to encourage the participants to enjoy an active aging life.
This program pay special attention to those frail elders and to those who suffered from pain which aims to reassure that elderly with different needs are welcome to join the exercises classes.
2S Stay Healthy
Physiotherapist will provide personal consultation to the elders in the NEC for an early intervention of their health problems. Through pain management and simple therapeutic exercises, the participants will be able to restore their independent living ability which can eventually reduce their needs for long term care.
3S Stay Safe@Home
Physiotherapist will meet the participants and caregivers in NEC or in their home, and access their difficulties in daily care and give suggestions for the proper transfer skills to release carers’ pressure.
We also host training sessions for the caregivers. Caregivers will be taught the caring skills, transfer skills , patients’ psychology, knowledge of disease and volunteering skills. Participants can practice during the classes to strengthen their skills and to increase their confidence in care.
